Abstract

The increasing prevalence of chronic diseases has created a growing need for continuous and coordinated healthcare services. Nurses play a central role in chronic disease management through patient education, medication support, lifestyle counseling, monitoring, and coordination of care. This paper examines the contribution of nurses to the management of chronic conditions such as diabetes, hypertension, cardiovascular diseases, and chronic respiratory disorders. Nurse-led interventions can improve patients' understanding of their conditions and encourage adherence to treatment and healthy lifestyle behaviors. Effective communication and individualized education can further strengthen patient self-management. Expanding the role of nurses in chronic disease management may improve continuity of care, patient engagement, and long-term health outcomes
