Abstract

Patient safety is a fundamental component of high-quality healthcare delivery. Medical errors, medication-related problems, communication failures, healthcare-associated infections, and inadequate clinical processes can adversely affect patient outcomes. This paper examines major patient-safety challenges and quality-improvement strategies in modern healthcare systems. Effective communication, clinical protocols, electronic health technologies, staff training, incident reporting, and continuous quality monitoring can reduce preventable harm. Developing a strong culture of safety requires collaboration among healthcare professionals, administrators, patients, and policymakers. Integrating patient-safety principles into routine healthcare practice can improve clinical outcomes, patient satisfaction, and overall healthcare quality..
